After initial contact with us, our care management team will be designing an individual personal care plan. We will meet with you and /or your representative to discuss your care before we commence .
We will contact you to arrange a convenient time to meet with you. During this meeting we will get to know you and your requirements and talk to you about the solutions we could provide.
During this meeting we will complete a care planning assessment plan if you decide we are the right care provider for you. This care plan will enable us to formulate a personalised care plan for your needs and wishes.
When you are completely satisfied with the care plan will we arrange a time for your care to commence and we will introduce you to your care team.
We review your care needs after the initial six months of commencing care and thereafter on yearly basis. We are however aware that your care needs may change any time and therefore we will conduct a review and update on changes of the services we provide anytime this happens. This will be done to ensure that you are satisfied with the level and quality of our support.
